Online registration for the fall conference is now CLOSED.

CHPRMS reserves the right to cancel or reschedule the Annual Conference or individual sessions due to unforeseen circumstances. Registration fees, less a $150 cancellation fee, are refundable if written notice is received by November 5, 2009. Cancellations of unpaid registrations also are subject to the $150 cancellation fee. After November 5, 2009, all fees/credits toward the conference are forfeited. Please direct written cancellation requests to CHPRMS, Attention: Judy Slocum, Medical Center Strategic Planning, Wake Forest University Baptist Medical Center, Medical Center Boulevard, Winston-Salem, NC 27157; alternatively, fax your cancellation request to 336.716.2879, or e-mail it to jslocum@wfubmc.edu. Please note that all refunds will be made to the payer only. No exceptions. You are encouraged to send a substitute if you cannot attend.
